How Quickly Can Hospital-to-Home Care Start?
Rapid Response: We aim to mobilise care within 24 to 48 hours of a request, depending on the complexity of the patient's needs. Quick intervention prevents gaps in support and promotes a faster transition to independence.

Our team is trained to assist with a range of complex post-operative needs, following the specific protocols provided by your consultant.

Collaborative Home Rehabilitation
Working With the MDT: We work in direct partnership with NHS physiotherapists, Occupational Therapists (OTs), and Dieticians. Our carers support prescribed exercise plans and mobility routines to ensure rehabilitation continues safely between professional therapy sessions.
Initial Home Assessment and Equipment
Prior to discharge, or immediately following, we conduct a nurse-led home assessment. While we do not source equipment, we work with community teams to identify the mobility aids or bathroom supports required, which are typically provided by social services and the NHS.
The Critical First 48 Hours
The first 48 hours following hospital discharge are the most critical for preventing readmission. Our Post-Operative team focuses on "Recovery Momentum" - ensuring that pain management is optimized so that you can begin gentle movement and rehabilitation as soon as your consultant allows.
We monitor for signs of infection, manage complex wound dressings, and ensure nutritional intake is sufficient for tissue healing, providing a safe clinical environment within your own four walls.
